By registering my child for the SOUL Purpose Soccer Clinic, I acknowledge and agree to the following:

1. I understand that participation in a soccer clinic involves physical activity and the risk of injury.

2. I hereby release, waive, and hold harmless the clinic organizers, Samantha Brand, coaches, volunteers, and any partnering facilities from any and all liability for injuries, damages, or losses that may occur during the clinic.

3. I confirm that my child is in good physical condition and has no known medical conditions that would prevent them from safely participating.

4. I authorize clinic staff to seek emergency medical care for my child if necessary and understand that I am responsible for any associated costs.

By checking the box on the registration form, I certify that I have read and agree to this waiver.
